No annual maximums, waiting periods, or deductibles
Your benefits run for one full year from the date this document is signed, must be used within that time, and are non-transferable to any other party. Unused services expire at the end of the term and cannot be rolled over. By signing below, you acknowledge that you are aware the benefits of this membership cannot be combined with insurance or any other discount plan, and that this membership is not an insurance plan. Your membership is non-refundable once any services have been rendered. If insurance benefits are obtained, this contract will become null and void with no refunds. All benefits you are entitled to are listed above, and no other benefits have been promised or implied. Discounted services extend only to those offered within this office, at the doctor’s discretion, and do not apply to any procedure referred to a specialist. The discounted rate is based on the office’s usual and customary rate (UCR) for specific procedures, which is subject to change. For additional discounted memberships, household members are defined as a spouse, domestic partner, child, or any individual living at the same physical address as you.
